Radiofrequency, commonly abbreviated RF, is a form of non-ionizing electromagnetic or electrical energy used throughout modern medicine.

Unlike X-rays or gamma rays, RF used therapeutically does not work by ionizing DNA.

Instead, most therapeutic RF devices produce their biological effects primarily by generating controlled electrical resistance and heat within tissue.

This simple principle has developed into remarkably different medical technologies.

Today, radiofrequency energy may be used for:

What Is RF Microneedling?

The concept is attractive because different depths can theoretically target different layers while limiting excessive surface heating.

A 2026 systematic review encompassing 41 studies found relatively consistent improvement in atrophic acne scars and reported benefits across several dermatologic indications.

However, RF microneedling deserves particularly careful attention because its power also creates risk.

An Important FDA Safety Warning

In October 2025, the U.S. FDA issued a safety communication regarding serious complications reported with certain uses of RF microneedling.

Reported adverse events included:

  • burns

  • scarring

  • loss of fat

  • disfigurement

  • nerve damage

  • injuries requiring medical or surgical treatment

The FDA specifically emphasized that RF microneedling is a medical procedure, not simply an ordinary cosmetic treatment, and recommended treatment by appropriately trained licensed healthcare providers.

Shortwave diathermy is another electromagnetic thermal modality related to RF technology.

A 2026 systematic review of 17 trials involving 1,372 people with knee osteoarthritis found improvements in measures of pain, walking, physical function, and stiffness in some comparisons. However, the authors emphasized that evidence certainty was limited, and shortwave diathermy did not clearly outperform ultrasound or electrotherapy across outcomes.

The study was small, so these findings should not be treated as universal success rates.

Acne scars

A 2025 systematic review of 16 studies involving 481 patients found fractional RF microneedling likely effective for facial acne scarring.

Facial rejuvenation

A 2026 systematic review of RF microneedling reported high patient satisfaction in many included facial-rejuvenation studies, although methodologies and devices differed considerably.

The responsible conclusion is:

RF can produce meaningful aesthetic improvement in appropriately selected patients—but it is not universally successful.

What RF Does Not Do
